roaccutane cancer is used primarily for severe cystic acne and<\/a> acne that has not responded to other treatments Many dermatologists also support its use for treatment of lesser degrees of acne that prove resistant to other treatments, or that produce physical or psychological scarringIsot retinoin is not indicated for treatment of prepubertal<\/a> acne and is not recommended in children less than 12 years of age<\/sup><\/p>\n

It is also somewhat effective for\u00a0hidradenitis suppurativa<\/a>\u00a0and some cases of severe\u00a0rosacea<\/a>.[15]<\/sup>\u00a0It can also be used to help treat\u00a0harlequin ichthyosis<\/a>,\u00a0lamellar ichthyosis\u00a0and is used in\u00a0xeroderma pigmentosum\u00a0cases to relieve\u00a0keratoses. Isotretinoin has been used to treat the extremely rare condition\u00a0fibrodysplasia ossificans progressiva. It is also used for the treatment of\u00a0neuroblastoma, a form of nerve cancer.<\/p>\n

Isotretinoin therapy has furthermore proven effective against\u00a0genital warts\u00a0in experimental use, but is rarely used for this indication as there are more effective treatments. Isotretinoin may represent an efficacious and safe alternative systemic form of therapy for\u00a0recalcitrant condylomata acuminata<\/a> (RCA) of the cervix. In most countries this therapy is currently unapproved and only used if other therapies failed<\/sup><\/p>\n

Prescribing restrictions<\/span>[<\/span>edit]<\/span><\/span><\/h3>\n

Isotretinoin is a\u00a0teratogen<\/a>; there is about a 20\u201335% risk for congenital defects in infants exposed to the drug\u00a0in utero<\/i>, and about 30\u201360% of children exposed to isotretinoin prenatally have been reported to show neurocognitive impairment.[18]<\/sup>\u00a0Because of this risk, there are strict controls prescribing isotretinoin to women who have potential to become (or be) pregnant while taking isotretinoin and many strongly advise to terminate their pregnancies because of the 20-60% risk.[18]<\/sup><\/p>\n

In the United States, since March 2006, the dispensing of isotretinoin is run through a website called\u00a0iPLEDGE<\/a>. The US\u00a0Food and Drug Administration\u00a0(FDA) requires the companies marketing the drug to put this website in place as a\u00a0risk evaluation and mitigation strategy. These companies formed a group called the Isotretinoin Products Manufacturing Group, and it hired\u00a0Covance\u00a0to run the website.[19]<\/sup>[20]<\/sup>\u00a0Prescribers, pharmacists, and all people to whom the drug is prescribed need to register on the site and log information into it. Women with child-bearing potential must commit to using two forms of effective contraception simultaneously for the duration of isotretinoin therapy and for a month immediately preceding and a month immediately following therapy. Additionally, they must have two negative\u00a0pregnancy tests 30 days apart and have negative pregnancy tests before each prescription is written<\/sup><\/p>\n

In most countries, isotretinoin can only be prescribed by dermatologists or specialist physicians; some countries also allow limited prescription by general practitioners and family doctors. In the United Kingdom[23]<\/sup> and Australia\u00a0isotretinoin may be prescribed only by or under the supervision of a consultant\u00a0dermatologist<\/a>. Because severe cystic acne has the potential to cause permanent scarring over a short period, restrictions on its more immediate availability have proved contentious.[26]<\/sup>\u00a0In New Zealand, isotretinoin can be prescribed by any doctor but subsidised only when prescribed by a vocationally-registered general practitioner, dermatologist or nurse practitioner<\/p>\n","protected":false},"excerpt":{"rendered":"
